Hi!

So I was in the kitchen one day and got a little excited with my new chef’s knife.

There I was chopping coriander, and my mind wondered… chop

The knife went through my finger diagonally. It mustn’t gone very deep as I felt no pain but there was profuse bleeding.

It also went through my nail and made a wide “U” at the top of the nail; but was still attached. It went into the skin of my thumb too.

After washing and picking out coriander (actually there was one bit left as I was scared to move the bit of nail too much! :/), I applied pressure, raised my hand and ran to the pharmacy down the road.

The pharmacist said it didn’t look too bad and as it was the weekend, I couldn’t visit the local surgery.

I applied antiseptic ointment (Savlon) and applied a plaster, and wrapped my finger with a waterproof white tape ‘dressing’ so I could continue cooking.

Now having set the scene my actual question(s) is/are as follows:

[*] I’ve been changing the dressing every 24 hours and the bleeding stopped after the first dressing. There is no pain but on the first day there was a dark haematoma-like bruising. So I washed the area again carefully and reapplied the ointment, plaster and bandage. These things are airtight and waterproof so should I be letting the wound to “breathe” or is this an old wives tale?

[*] Is it normal for the skin that has been covered for 24 hours (on several occasions) to develop a lighter/wrinkled hue to the surrounded skin? In some areas (for example I assume where the ointment has collected under the bandage) the area is extremely white and in contrast with the other areas around it. Is this just because of saturation or is it a sign of infection?

I am on a very very busy schedule and I would be amazingly grateful if I could get feedback on my situation. I look forward to your replies.

I panicked at Jessica’s reply but I’m a little more relaxed now with Butterfly’s other end of the spectrum reply… but to be safe I may just pop down to the surgery and they’ll send me to the ER if necessary.

Thankfully its my left (non-dominant) hand and therefore I can try my best to keep it out of harms way during the day. The bandage/dressing I applied was originally in fear of squashing/deforming the wound as I slept!

I’ll continue to monitor this thread and thanks again for the replies!
Oh I forgot to mention that the wound itself has cleared up nicely, the bruising has disappeared, there is a little flappy skin and the cut part of the nail has reseated itself like a jigsaw and holds in place without the bandage. Pressing around the nail and on the actual flap of skin does not sting or hurt. There is no secretion and the only cause for concern was the slightly too-long-in-the-warm-bath look of some bits of skin in the covered top section of the thumb (and not on the actual wound/around the wound itself).

I hope the above paragraphs weren’t too incoherent!

First off, you should have gone to the emergency room! You only have a few hours in which to get suture before the wound begins to heal. If you do need sutures they will have to reopen the wound and suture it. You should go to a doctor anyway as this skin flap could die if the blood is not circulating well and you could end up with a huge infection. If you need to clean it, use 50% luke warm water and 50% betadine solution. Skin is usually kept covered but not with a bandaid (usually wrapped in a sterile gauze). The bandaid can stop the air flow and cause skin to die around the edges. You also shouldn’t be purring ointment unless directed by a doctor because not all ointments are the same.

Question by : This has to do with wound care. A friend of mine has an open wound possible caused by a water bubble that open?
He has diabetes and is taking insulin to control it. Some times his blood sugars are high but most of the time it is in the normal range. It started out as a small water bubble when it broke open about the size of 1/2″ to 2″, now it is about 3″ long and 2″ wide plus his ankle is swollen and just below the wound it is all red. He has pain when touching the wound but not around it. What should I do for him take him to a different doctor or just let the treatment run its course. Someone please help….THANKS IN ADVANCE.
Treatment now: cleaning wound with sterile water and betadine and applying a white cream ( I don’t know what it is) then wrapping it with gauze and an ace bandage. Also, applying ice when possible and keeping leg raised.

Foot and lower leg wounds are particularly troublesome for diabetics. Their circulation is compromised as a general rule and especially in those areas furthest from the heart. So this wound needs vigilant treatment and observation. What you’re describing is cellulitis which just means the inflammation in the cellular structure under his skin.

I imagine the white cream you’re using may be silvadene which is commonly used in burns. Even though your friend may not have encountered a heat source the wound you describe sounds like a burn. It is very important to continue the treatment as long as the doctor indicates. I really think your friend’s doctor is doing the right things and see no real reason to seek another MD. It’s going to be most important to elevate that leg as much as possible … at work, while riding in a car, just while sitting around watching TV or at meal times. It simply improves the circulation and that’s what will assure the wound heals successfully.

Tell your friend to take SUPER good care of his feet!! He should use a good quality foot cream every night. When he bathes or showers he needs to be very particular about drying his feet; between the toes, along the toenail cuticles … all that needs daily attention and then the cream needs to be applied and rubbed into the skin until it’s all absorbed. This will keep the skin on his feet supple and he can expect less problems with sores or blisters. Should he start a day with ill fitting shoes he should NEVER finish the day with them. Tell him at work to keep a pair of those socks that have treads on the bottom in case he finds himself in that situation.

You don’t need to make a huge deal of this, but simple foot wounds lead to amputations in diabetic patients. Not a good outcome!

The only one I’ve used is Jergens Natural Glow moisturizer. It works, but it still smells like sunless tanning stuff, though not as bad as some of the other products out there.

I have fine lines around my eyes and am starting to see them around my mouth… I don’t mind paying a lot for it, if it actually works. I’m not looking to make the lines disappear, i know that’s probably not possible, I am 38. But to get a softer looking skin is my goal. I’ve looked at oil of olay products, and a few others on the market, but I want to get a good one.

I would prefer answers from people who have actually seen results in the product they are using. Thanks

Question by : Puncture wound care on a horse?
my horse on thursday (yesterday) had some how out on pasture injured his rear left leg, on the front part more trowds the bone and tendon there. (i looked it up and its around the lateral digital extensor tendon) its not that deep but i guess still deep enough that it has made the whole lower leg swollen and hot. i need to know how to take further care of this injury. so far iv removed the dirt and dust from it and genitally sprayed the whole leg with hose water. i have also put on iodoform oil.
what else do i need to do and how to i let it properly drain.
ok thanks and please do not tell me to call the vet becasue i am calling in tomorrow afternoon.

have you wrapped it? i would wrap it and put some corona or underwood’s on it or have you tried vetericyn? i’ve heard that works well. i might also put a cold thing under the wrap, but not directly on it. i hope your horse gets well soon

my husband got a cut in his arm. we went to the er and got stitches. we took the stitches out when they said we could. within hours the wound was open again. it had not healed at all! we do not have insurance and do not want to have another bill from the er. is there anything i can do here to help the cut heal? we were trying to think of some way to do at home stitches…i did find some fishing line that is almost identical to what was used in the er….but i can not find a needle. anyone know where i can find a small needle? i also got some liquid bandage…will this work for it or will it take too long to heal? butterfly bandages will not hold it together. any ideas?

You can usually wash an area that has been stitched in one to three days. Washing off dirt and the crust that may form around the stitches helps reduce scarring. If the wound drains clear yellow fluid, you may need to cover it.

Your doctor may suggest that you rinse the wound with water and rebandage it in 24 hours. Be sure to dry it well after washing. You may want to keep the wound elevated above your heart for the first day or two to help lessen swelling, reduce pain and speed healing.

Your doctor may also suggest using a small amount of antibiotic ointment to prevent infection. The ointment also keeps a heavy scab from forming and may reduce the size of a scar.

Stitches are usually removed in 3 to 14 days, depending on where the cut is located. Areas that move, such as over or around the joints, require more time to heal.

Deep cuts can take a really long time to heal, so be patient. Try an antibiotic ointment that is also a pain reliever, that should help with the pain and also prevent infection. Neosporin can include a pain reliever if you get the right kind. Then cover it with a non-stick gauze pad and change it when the cut starts to bleed through. You could even wrap an Ace bandage around your leg over the gauze, for some added pressure to stop the bleeding. That has worked for me before.

And it’s too late to try this now, but in the future maybe use butterfly strips? Or get stitches if it’s a really serious cut.
